If you’re a school looking to receive funds given to you through a donation, fill out the form below!

Email Clayton Hackler – clayton.hackler@davecampbells.com – your district W-9 form with any additional information (additional forms needed to be filled out, specific departments receiving the funds, etc.) and he will process your check request!

NOTE: All donation forms received BY Friday, February 13th, 2026 will be distributed within 1 month of submission.

Any forms not submitted by the deadline MUST be submitted by Friday, March 27th, 2026 and will be distributed by the end of the 2025-26 school year.
